To Apply for a passport book or passport card,
you must have the following items: |
|
|
|
|
|
- |
Passport application -
completely filled out in BLACK INK but NOT
signed. If areas are left blank or have inaccurate
information, it may cause your passport to be delayed or
refused. If you use any other color than black ink, you
will have to redo your passport application. |
|
|
- |
Passport photo (can be obtained
from Walgreen Drug, Wal-Mart, or Hy-Vee) |
|
|
- |
Certified birth certificate
(not the hospital certificate and not a photocopy) or previous
U.S. Passport (for an adult only) or naturalization certificate. |
|
|
- |
Valid driver's license or another
form of current picture identification. |
|
|
- |
Two separate checks or money
orders are required for each passport application and the
fees are: |

Passports for children 16-17 need one parent
present to sign the application along with proper
identification. |

|
|
|
All passport applicants, regardless of age,
must be present to apply. |

|
|
|
|
Mower County does not accept debit cards or
credit cards. |
|
|
|
|
|
|
