The Community Health Assessment (CHA) is a collaborative process between community health boards and the community itself, providing us with a look at our county’s overall health and well-being. Through multifaceted outreach efforts, our team assessed the predominate social determinants of health and top barriers faced by our residents that prevented them from living their healthiest lives. This data will be used to produce the Community Health Improvement Plan (CHIP), as an action plan to mitigate the priority areas of concern. This report is projected to be published in October 2025.
The CHA and CHIP are about what we can do as a community, together, making our community healthier, stronger, safer, and more accessible for everyone who calls Mower County their home.
