Assessments

Services designed to support the elderly as well as people with disabilities; in their efforts to remain living as independently in the community as possible. There are several different programs that are targeted to specific age groups and specific types of disabilities. A MnCHOICES assessment is a service provided by the county to assist individuals in determining what services an individual may be eligible for. A social worker or nurse will conduct the assessment with the client and family at the client’s residence.

The assessor can then provide directions and information regarding resources based upon the client’s assessed needs and eligibility. The client, family, and assessor can then discuss options regarding a care plan, taking into consideration all aspects of the client’s unique situation and what is important to and for them. This is a free service.  

Assessment Referral 

The subject of each referral will be contacted to confirm that these are services desired by the individual. In order to access any programs or services through a MNChoices Assessment, the individual will need to have medical Assistance; therefore, we ask that prior to scheduling your MnChoices assessment you either have medical assistance or have turned in a MA application to ensure that your MNChoices assessment has value. Here is a list of the medical records we ask individuals to provide:

  1. Medical records for the last 24 months which include the following information:
    • Current list of all medical diagnoses, with ICD-10 codes.
    • List of current medications and immunization records.
    • Medical History; most recent physical notes; health case notes; and treatment records.
    • physical therapy/occupational therapy notes.
  2. Copies of a recent Psychological Evaluation with Full-Scale IQ; Adaptive Functioning, and Psychiatric testing reports, etc.
  3. Most recent school Individual Educational Program (IEP), and Evaluation Report.
  4. Care plan and/or treatment plans from community established providers.

Alternative Care Program

A Home and Community Based Services Waiver that provides home and community based services to people who need nursing home level of care but choose to live in the community. The Alternative Care program (PDF) provides many of the same services as the Elderly Waiver program.  Alternative Care is for people with low income and assets who are not eligible for Medical Assistance.

Brain Injury Waiver

The Brain Injury Waiver (PDF) provides funding for home and community-based services for children and adults who have an acquired or traumatic brain injury. 

Case Management Services

Case Management Services assist persons on a waiver with accessing needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source.

Community First Services & Supports (CFSS)

Community First Services & Supports is a Minnesota Health Care Program that offers flexible options to meet the unique needs of people.  CFSS allows people greater independence in their homes and communities replacing Personal Care Assistance (PCA) and the Consumer Support Grant (CSG).

Community Alternative Care Waiver

The Community Alternative Care Waiver (PDF) provides funding for home and community-based services for children and adults who are chronically ill.  The Community Alternative Care Waiver is designed to serve persons with disabilities who would otherwise require the level of care provided in a hospital.

Community Access for Disability Inclusion Waiver

The Community Access for Disability Inclusion Waiver (PDF) provides funding for home and community-based services for children and adults, who would otherwise require the level of care provided in a nursing facility.

Developmental Disabilities Waiver

The Developmental Disabilities Waiver (PDF) provides funding for home and community-based services for children and adults with developmental disabilities or related conditions.

Elderly Waiver

The Elderly Waiver (PDF) provides funding to seniors to help pay for services so they can stay in their homes.

Essential Community Supports

Essential Community Supports (PDF) is a program that provides funding for some services that many seniors first find helpful such as homemaking or meals.

Family Support Grant

Family Support Grant (PDF) is a program that provides state cash grants to families of children with certified disabilities.

Moving Home Minnesota

Moving Home Minnesota (PDF) is a program that assists people with transitions from nursing homes and other qualified institutions to community living.

Semi-Independent Living Services

Semi-Independent Living Services maintain and improve the capacity of an adult with a developmental disability to live in the community.
