Home & Community Based Waiver Services

Assessments

Services designed to support elderly and people with disabilities in their efforts to remain living as independently in the community as possible. There are several different programs that are targeted to specific age groups and specific types of disabilities. A MnCHOICES assessment (PDF) is a service provided by the county to assist individuals and their families in planning and choosing services. A social worker or nurse will conduct an assessment with the client and family at the client’s residence.

The assessor can then provide direction and information regarding resources based upon the client’s assessed needs and eligibility. The client, family, and assessor can then discuss options regarding a care plan, taking into consideration all aspects of the client’s unique situation and what is important to and for them. This is a free service.

To schedule an assessment, email Home and Community Based Services Intake or call 507-437-9744.
