Mower County Aging Services provides case management / care coordination for individuals that are 65 and older. Programs include Elderly Waiver (EW), Alternative Care (AC), Essential Community Services (ECS) and CFSS (previously PCA). The plans we work with are Fee-For-Service, Blue Plus, UCare, and Medica. For those 65 and older and on a health plan, a care coordinator is automatically assigned. Age 65 and older not on a health plan can contact our intake worker to apply at 507-437-9744.
A Care Coordinator conducts an initial assessment to determine if someone is eligible for services. If eligible, the care coordinator will assist the member with finding and/or setting up services and supports to best meet their needs and will also complete annual reassessments and mid year check ins.
